Dear Patient,
We hope this letter finds you and your family in good health. The COVID-19 pandemic has brought many changes to the
dental community, but one thing has remained the same: All of us at Amidi Family and Cosmetic Dentistry are
committed to your safety and health.
In preparation for reopening based on Santa Clara County’s guidelines, we want to tell you about the infection
control procedures we follow in our practice to keep patients and staff safe. Our office follows infection control
recommendations made by the American Dental Association (ADA), the U.S. Centers for Disease Control and Prevention
(CDC) and the Occupational Safety and Health Administration (OSHA).
We have made some changes to help protect our patients and staff. For example:

Staff members will take a COVID-19 test weekly; we will take temperature checks daily. Any staff members
experiencing flu-like symptoms will be required to stay home and not come to work.

We will call you before your appointment to ask some questions to ensure you are not sick. You will be asked
these questions again when you arrive in the office.

We will cancel the appointments of any patients with COVID-like symptoms such as fever, chills, coughing,
sneezing or loss of taste and smell. We will also cancel the appointments of any patients who have been in
contact with a COVID-19 patient within the past 2 weeks. If you have been diagnosed or have been in close
contact with someone diagnosed with COVID-19, we will request physician clearance before rescheduling your
appointment.

When you arrive for your appointment, please call the office (650) 988-6500 from
your car to make sure we are ready for you before you come inside. Any companions need to wait outside the
office during your appointment.

Please cover your face with a mask and/or cloth when entering the office. As you step inside, please use the
sanitizing floor mat to clean the bottom of your shoes.

You will be greeted by our staff, who will be wearing personal protective equipment (PPEs). This consists of an
N95 mask, face shield, fluid-resistant gown and rubber shoes.

We will ask you to use hand sanitizer when you enter the office.

We will take your temperature with a touchless thermometer. Anyone exhibiting an elevated temperature will be
rescheduled.

As we take you back to an operatory for your appointment, we will ask you to rinse your mouth with a
disinfectant.

Before seeing each patient, all dentists, hygienists and dental assistants will put on a new gown, gloves, N95
mask and face shield, as well as disinfect their hands.

We have increased the time for each service, generally 90 minutes. Please make sure to allocate for this
additional time in your schedule.

Appointments will also be managed to allow for social distancing between patients.

Your procedure will include utilizing a new Isolite dental isolation system and a new high-suction power unit
(ADS)—to remove droplets and aerosols, enhancing the safety of patients and staff.

We have installed a UV-C light unit in the ductwork of our central air system to scrub the air as it passes by,
eliminating viruses and bacteria. We also have surgical-grade air purifiers and portable air filtration units
that clean the office air throughout the day.

We will clean all the treatment-room surfaces after each patient with hospital-grade disinfectants that kill
99.9% of bacteria, spores and viruses. We will regularly clean the lobby and reception room, bathrooms, door
knobs and seat handles with disinfectants. And we have installed sneeze shields in the reception area.

After your appointment, you will exit from our rear door.
We hope that this communication will make you feel comfortable about your future visits with us. We will do
everything we can to protect you while you are in our care. We are happy to answer any questions you may have about
the steps we are taking to keep you and every patient in our practice safe.
